Long-Term Care

Radiant Care Tabor Manor’s Long-Term Care home accommodates up to 128 individuals in private or basic rooms, with 24 hour nursing care provided by qualified staff, and supervised by Registered Nursing staff.

Our Long-Term Care home, opened in August 2013, is located on the north end of our Tabor Manor property and is 4 storeys high with 4 resident home areas, each serving 32 residents. Amenities include sunny dining rooms, a courtyard garden, a full range of activities that contribute to the health and well-being of our residents, and a Chaplaincy program that provides church services, visitation and special programs to meet the spiritual needs of residents. Radiant Care Tabor Manor is funded through the Ontario Ministry of Long-Term Care. Our home is reviewed regularly to ensure that the standard of care is in compliance with ministry standards.

All applications must be processed through Ontario Health atHome. Call 310-2222 for more information and assistance in completing an application. There is no area needed, the service will automatically direct your call to the nearest office from your calling location.

To report a complaint, one can call the Ministry of Long-Term Care Family Support and Action Line at 1-866-434-0144.

Apartment Living

We offer a number of meeting/activity rooms, lounges, and sitting areas throughout our campus to provide welcoming spaces for tenants to socialize and entertain family and friends.

Mapleview Apartments

Mapleview is a four-storey building of 62 rental units, including 58 one-bedroom and 4 two-bedroom apartments. All apartments enjoy full kitchen, bathroom, living room, bedroom(s), balcony, and ample storage space. Four of the units are designated as accessible for individuals with disabilities.

Evergreen Apartments

Evergreen opened its doors in 1994, with 58 one-bedroom and 4 two-bedroom apartments. All apartments enjoy full kitchen, bathroom, living room, bedroom(s), central air, and ample storage space.

Wellness Suites

Spruce Lane Wellness Suites opened in 2015, with 8 one-room and 17 two-room units. Each unit is comprised of a bed-sitting room, closet, and two-piece washroom. Two-room units also include a living room. Wellness Suite tenants enjoy three meals daily in the Dining Room and receive assistance with activities of daily living from certified personal support workers. A lounge in the south wing, a centrally located Olive Branch Café, and an entrance sitting room offer opportunity for socializing/entertaining.

Supportive Housing

Supportive Housing Services are available to any individual residing at Radiant Care Tabor Manor who is assessed to be eligible. With funding from the Ministry of Health, personal care can be provided to those who require it, and assist them with their daily needs.

Services might include basic housekeeping, laundry, 24 hour safety checks, medication reminders and assistance with bathing and other personal care.

Supportive Housing is designed to help people remain independent, living in their own apartments for as long as possible.

Once you have moved in to your apartment unit, life lease, or wellness suite, one of our Supportive Housing staff will visit you to set up your individualized program of services, as you are eligible.

Tours

Long-Term Care

If you wish to book a tour of our long-term care home, please contact our Administration Office for the next available date and time. (Please note that if our home goes into outbreak, tours will be cancelled and rescheduled). If you/your loved one is offered a bed in our long-term care home through Ontario Health atHome, we will accommodate a tour and a viewing of the room before your make your decision, if you wish.

Apartments & Wellness Suites

We do not offer tours of our apartments and wellness suites. If you are offered an apartment or wellness suite at our home, we will arrange a time for you to view the unit before your make your decision, if you wish.

Hospitality Suites

Radiant Care Tabor Manor offers temporary accommodation to guests and visitors by making available 6 Hospitality Suites. Each suite includes sleeping accommodation for three to four persons, and ensuite. There are four hospitality suites in Evergreen and two in Mapleview.
